Information collected

The inquiry form collects the name, email address or telephone number, relationship to the inquiry, message and submission date and time that a visitor chooses to provide. Basic request information may also be processed to keep the website secure and prevent spam.

Information is used to review and respond to general inquiries, maintain website security, prevent spam, document communications and comply with applicable legal obligations.

Where submissions go

Submissions are processed through the website and stored in private storage provided by Vercel for review by authorized Dignity Living personnel. The ordinary inquiry form does not currently send email notifications.

Service providers

Vercel provides website hosting, form processing and private storage. Dignity Living uses these services to operate the website and respond to inquiries. No advertising, session-replay or public-site analytics service is configured.

Cookies and analytics

The public website does not use analytics or advertising cookies. An essential secure session cookie may be used in a restricted administrative area.

Access and sharing

Inquiry information may be accessed by authorized Dignity Living personnel and service providers only as needed to operate the website, protect it and respond to the inquiry. Dignity Living does not sell inquiry information. Information may also be disclosed when required by law or to protect rights or safety.

Retention and deletion

Website inquiry records are generally retained for up to 24 months unless a shorter period is requested or a longer period is reasonably required for an active matter or legal obligation. Verified deletion requests will be applied to Dignity Living’s records and, when appropriate, communicated to relevant service providers.

Security

Dignity Living uses reasonable administrative and technical safeguards, including HTTPS, private storage, access controls, input validation and spam protections. No internet transmission or storage system can be guaranteed completely secure.

To request access to, correction of or deletion of website inquiry information, email support@dignityliving.com or call 775-377-7676. Dignity Living may need to verify the request and may retain information when required by law.

Do not send health records, IPPs, Social Security numbers, financial information, medical details or other sensitive information through the ordinary contact form. Dignity Living does not claim that this ordinary form is HIPAA compliant.
